Northwich Primary Care Network (PCN)
Northwich Primary Care Network (PCN) is the name given to the organisation of GP practices who are working together to provide better, more co-ordinated, effective and sustainable care for the people who live in Northwich.
Northwich PCN consists of seven practices across eight locations, these are:
- Danebridge Medical Centre (Including Kingsmead)
- Firdale Medical Centre
- Middlewich Road Surgery
- Oakwood Medical Centre
- Watling Street Surgery
- Weaverham Surgery
- Witton Street Surgery
By working together and sharing resources, we are employing more skilled staff than ever before so that we give people the right care at the right time.
Together, we are responsible for the care of over 70,000 people in Northwich and the surrounding areas, working to create a better health future. The PCN consists of all the teams who work in these surgeries; we also have some staff who work across the whole of the PCN.
Cheshire and Merseyside Integrated Care Board (ICB)
