You have a right to access your health records, subject to certain conditions. If you want to see information that is kept about you, please make a written request to the practice manager.
You are able to have secure on-line access to your medical records via EMIS ACCESS. If you would like detailed coded access to your records you must make a separate application to Reception and it requires by law you having two forms of ID one of which is photographic. Your application can take some time, and you may be denied if your records contain sensitive information. Please make initial enquiries at Reception.
